Participation and risk. I am the player's parent or legal guardian, have authority to consent, and permit participation in the sessions listed above, including their warmups, instruction, drills, and games. Hockey involves falls, collisions, physical contact, flying pucks, sticks, sharp skate blades, equipment failure, and exertion. These can cause concussion, dental injury, broken bones, permanent disability, or death. Equipment and supervision cannot eliminate these risks. I understand these risks and will discuss safety with my child. This agreement does not cover unlisted future sessions or another organizer's activities.
Safety and emergency assistance. My child must wear required protective equipment, follow safety instructions, and report injury or illness immediately. NICE may stop participation for safety or misconduct. I will privately notify NICE before participation of relevant restrictions or requested accommodations. A player with a suspected concussion must stop participation pending appropriate evaluation and clearance under applicable requirements. I authorize NICE to contact me, summon emergency services, and request emergency evaluation and treatment by qualified professionals if I cannot promptly be reached, to the extent permitted by law. This is not medical clearance or a release of liability for treatment.
RELEASE OF THE PARENT'S OWN CLAIMS — PLEASE READ. To the extent Illinois law permits, I release NICE and its officers, directors, employees, coaches, and volunteers, acting in their program roles, from claims belonging to me personally arising from their ordinary negligence in conducting the listed activities, including my own claims arising from my child's participation or injury. This does not release my child's claims, require me to indemnify anyone for those claims, or release willful and wanton conduct, intentional misconduct, or liability that cannot lawfully be released. Illinois law governs, subject to mandatory applicable law. If a provision is unenforceable, the remaining provisions apply to the extent permitted by law.
Booking terms. Paid skater spots are confirmed after payment; goalies skate free. Waitlisting does not reserve a spot. Payments are non-refundable for parent cancellations or no-shows. If NICE cancels, credit applies to a future skate, subject to applicable law. Material changes will be communicated before participation. These terms do not waive mandatory consumer rights.

Optional photo/video permission — separate choice
- [ ] Yes: I authorize NICE to use my child's photographs/video from the listed sessions on its website, social media, and promotional materials without compensation. This excludes separate publication of the child's full name, school, or contact details, sale to third parties, and misleading or inappropriate uses.
- [ ] No: I do not authorize promotional use. Participation is unaffected.
Neither choice is preselected; no permission is treated as “No.” Permission may be withdrawn for future use at nicehockey312@gmail.com. NICE will stop new uses and reasonably remove material from channels it controls; prior printed distribution and third-party copies may not be recoverable. This covers NICE's use, not independent spectators or organizers.
